O GH MELHORA A RESISTÊNCIA ORGÂNICA E PROPICIA UM MELHOR CRESCIMENTO LINEAR OU LONGITUDINAL, AUMENTO DE PESO, AUMENTO DE MASSA MUSCULAR, AUMENTO DA DENSIDADE MINERAL ÓSSEA EM CRIANÇA, INFANTIL, JUVENIL E ADULTOS COMPROMETIDOS. FISIOLOGIA–ENDOCRINOLOGIA–NEUROENDOCRINOLOGIA-GENÉTICA–ENDÓCRINO-PEDIATRIA (SUBDIVISÃO DA ENDOCRINOLOGIA): DR. JOÃO SANTOS CAIO JR. ET DRA. HENRIQUETA VERLANGIERI CAIO.

Embora o FDA tenha aprovado o tratamento com GH em adultos e crianças com doenças crônicas é considerado “off-label”. As crianças com doenças crônicas frequentemente apresentam velocidade de crescimento diminuída em comparação com seus pares saudáveis de mesma idade (Saavedra et al. 1995). A terapia com GH aumentou a altura e o peso em crianças com doenças crônicas. Vários estudos (Alemzadeh et al. 1998; Hardin et al. 1998, 2001, 2005a), incluindo dois grandes ensaios multicêntricos (Hardin et al. 2006; Schnabel et al 2007), demonstraram boa melhora no crescimento linear, no peso corporal, na massa magra e no conteúdo mineral ósseo e na redução das internações hospitalares em crianças com fibrose cística. Na Europa o GH foi aprovado para reverter insuficiência do crescimento mediada por esteróides para pacientes com artrite inflamatória juvenil. Não só o GH diminuiu a perda de velocidade de crescimento destas crianças, mas também promoveu o acréscimo do conteúdo mineral ósseo e aumento da massa muscular (Bechtold et al. 2005). Estudos em crianças com crescimento deficiente devido ao tratamento com glicocorticóides por qualquer razão demonstrada, em média, a taxa de crescimento foi o dobro em resposta ao rhGH (Allen et al. 1998). Finalmente, o GH aumentou significativamente a massa livre de gordura, a deposição mineral óssea, o ganho de peso e o crescimento linear, sem afetar negativamente a atividade da doença ou tolerância a carboidratos em crianças que têm a doença inflamatória intestinal (DII) (Mauras et al. 2002). A doença crônica em crianças é frequentemente associada com o crescimento pobre, que pode estar relacionado com os distúrbios subjacentes, à má alimentação, ou à administração de drogas terapêuticas inibidoras do crescimento. 

Embora os efeitos benéficos do GH possam ser importantes para indivíduos com essas condições, os endocrinologistas precisam determinar a adequação do tratamento com GH em uma base individual de paciente para paciente e prescrever a droga "off-label".

Although the FDA has approved GH treatment in adults with AIDS and in HIV-infected children it is considered "off-label". HIV-infected children often have diminished rate compared to their healthy peers of the same age (Saavedra et al. 1995) growth. This decline in growth was significantly correlated with the viral load (Johann-Liang et al. 2000 Arpadi et al. 2000). GH therapy increased the height and weight in children with HIV infection, but has not been associated with an increase in viral load (Hardin et al. 2005b). Several studies (Alemzadeh et al. 1998, Hardin et al. 1998, 2001, 2005a) includes two large multicenter trials (Hardin et al 2006; Schnabel et al. 2007), showed good improvement in linear growth, body weight, lean mass and bone mineral content and reducing hospital admissions in children with cystic fibrosis. In Europe GH was approved to reverse the impairment mediated with steroids for patients juvenile with idiopathic arthritis. 

Not only the loss of GH decreased growth velocity of these children, but also promoted the increase in bone mineral content and increased muscle mass (Bechtold et al. 2005). Studies in children with impaired growth due to treatment with glucocorticoids demonstrated for any reason, on average, the growth rate was twice that in response to rhGH (Allen et al. 1998). Finally, GH significantly increased free-fat mass, bone mineral deposition, weight gain and linear growth without negatively affecting disease activity or carbohydrate tolerance in children who have inflammatory bowel disease (IBD) (Mauras et al. 2002). Chronic illness in children is often associated with poor growth, which may be associated with underlying disorders, poor feeding, or the administration of therapeutic drugs inhibiting growth. Although the beneficial effects of GH may be important for individuals with these conditions, endocrinologists need to determine the suitability of GH treatment on an individual basis from patient to patient and prescribe the drug "off-label".
